Negli ospedali di tutta Europa, l’infezione da Clostridioides difficile (CDI) rimane una sfida persistente e costosa. Nonostante decenni di esperienza clinica e diagnostiche sempre più sofisticate, identificare con precisione chi è effettivamente affetto da CDI e chi non lo è rimane una sfida sia per i clinici che per i laboratori di microbiologia. Al centro della questione c’è una verità fondamentale: diagnosing CDI is not a single test decision, but a clinical process that demands nuance, context, and stewardship 1.2.
Recent discussions show that even well-intentioned diagnostic strategies can lead to both underdiagnosis and overdiagnosis, directly impacting patient outcomes, infection control, and antibiotic use1.
Quando la sensibilità non è tutto
I test molecolari, in particolare i NAAT, hanno trasformato la diagnostica della CDI. NAATs offer greater sensitivity than toxin enzyme immunoassays or GDH screening, making them central to many European testing algorithms3. However, factors beyond sensitivity also require attention.
La sola sensibilità non garantisce certezza clinica. Un risultato molecolare positivo conferma la presenza di C. difficile tossigenico, ma non sempre indica un’infezione attiva. La colonizzazione asintomatica è comune nelle strutture sanitarie. Without careful interpretation, sensitive tests may identify the organism even when it is not causing disease4.5.
Le insidie nascoste degli algoritmi multifase
Per bilanciare accuratezza e praticità, molti laboratori si affidano a percorsi diagnostici multifase che combinano GDH, test delle tossine e NAAT. While these algorithms aim to improve predictive value, they introduce their own vulnerabilities 6.7.
Lo screening con GDH, ad esempio, può produrre sia falsi negativi che falsi positivi. Alcune specie di Clostridium e persino organismi non correlati possono causare reazioni crociate, mentre alcuni ceppi tossigenici possono sfuggire completamente alla rilevazione. Toxin assays, meanwhile, are prone to variability and can generate misleading results when used in isolation6.8.
Alcuni ceppi sfidano le ipotesi tradizionali. Alcuni C. difficile isolati non hanno le tossine A o B, ma sono portatori dei geni della tossina binaria. Without accounting for these nuances, diagnostic tools may miss or delayed clinically significant cases 6.9,10.
Cosa ci dice l’evidenza del mondo reale
I dati clinici emergenti stanno rimodellando il modo in cui i laboratori pensano alle strategie di test della CDI. Gli studi che valutano algoritmi inversi in due fasi e approcci NAAT autonomi mostrano benefici tangibili: fewer hospital onset CDI cases, reduced antibiotic exposure, and more consistent infection control practices5,11.
Sulla base di queste informazioni, una scoperta rilevante è che alcuni pazienti NAAT-positivi possono risultare negativi allo screening GDH. Clinically, using GDH alone as a gatekeeper may result in missed diagnoses, allowing unidentified cases to contribute to hospital transmission5.
Viceversa, trattare ogni positività molecolare senza considerare sintomi o fattori di rischio comporta il rischio di un uso non necessario di antibiotici. Questo rafforza un messaggio centrale: laboratory results must be interpreted through a clinical lens, not in isolation4,12.
L’imperativo della gestione responsabile
Man mano che la diagnostica diventa più potente, cresce la responsabilità di usarla con saggezza. Diagnostic stewardship, the principle of ordering the right test for the right patient at the right time, is now recognized as an essential partner to antimicrobial stewardship1.2.
Nella CDI, per gestione responsabile si intende evitare i test reflex in situazioni a bassa probabilità, evitare test ripetuti senza giustificazione clinica e integrare il supporto decisionale nei flussi di lavoro di richiesta. Evidence from hospitals implementing stewardship have been associated with reductions in unnecessary testing and false‑positive results without clear evidence of increased missed diagnoses.13,14.
Ciò è particolarmente rilevante con il crescente uso di pannelli gastrointestinali multiplex. Oltre a identificare rapidamente una vasta gamma di agenti patogeni, questi strumenti aumentano anche la probabilità di rilevare la colonizzazione C. difficile piuttosto che l’infezione. Studies indicate that only a minority of panel positive results ultimately represent true CDI, yet many patients still receive treatment5,15.
Il futuro: integrazione, non isolamento
Il futuro della diagnosi di CDI non risiede in un singolo test “perfetto”. Instead, it depends on integrating advanced molecular diagnostics with clear institutional policies, clinical judgment, and stewardship frameworks 1,16.
I laboratori e i medici devono collaborare per definire quando è appropriato il test, come devono essere interpretati i risultati e in che modo i percorsi diagnostici si allineano agli obiettivi di gestione del paziente. This integrated approach helps avoid harm, reduces unnecessary antibiotic exposure, and supports better infection control decisions2.
In definitiva, migliorare la diagnosi di CDI non significa solo rilevare i batteri. Significa comprendere la malattia. And in an era of powerful diagnostics, that understanding has never mattered more1.
