Study Shows Xpert® Xpress MVP Testing Increases Likelihood of Appropriate Treatment

Women seeking care for vaginal symptoms often present with similar complaints: discharge, itching, odor, irritation, or discomfort, but the underlying cause is not always clear. Distinguishing between bacterial vaginosis (BV), vulvovaginal candidiasis (VVC), Trichomonas vaginalis, or no infection at all can be challenging using traditional diagnostic approaches.

A newly published randomized clinical trial# provides important evidence that rapid molecular testing performed at the point of care using Xpert Xpress MVP (multiplex vaginal panel) can help clinicians make more informed treatment decisions. Women tested with Xpert Xpress MVP were significantly more likely to receive appropriate treatment within 24 hours than those managed with usual care.

A persistent diagnostic gap:

  • Approximately 25% of providers reported relying primarily on symptoms or visual assessment to guide treatment decisions.
  • Standard approaches correctly identified infections only about 50–63% of the time.
  • More than 1/3 of symptomatic women ultimately had no detectable bacterial vaginosis, Candida, or Trichomonas infection.

 

When symptoms overlap and traditional diagnostic tools have limitations, treatment decisions can become more difficult.

Diagnostic Blind Spot

Getting the diagnosis right matters. When infections are missed, women may not receive appropriate care. However, when treatment is prescribed in the absence of infection, patients may be exposed to unnecessary medications and the associated risks. The study examined whether molecular testing using Xpert Xpress MVP could impact appropriate treatment decisions at the point of care.

Hillier and colleagues# randomized women presenting with vaginitis symptoms to one of two diagnostic pathways:

ARM 1: Usual care, including symptom assessment, microscopy, and routine diagnostic practices

ARM 2: Point-of-care molecular testing using the Xpert Xpress MVP test

 

Among women with confirmed vaginal infections:

  • 89,6% of women tested with Xpert Xpress MVP at the point of care received appropriate treatment within 24 hours
  • Only 51,9% of women received appropriate treatment within 24 hours when managed using usual care approaches
  • Among women with no detectable vaginal pathogens, 50,0% received unnecessary treatment with usual care compared to 27,1% with Xpert Xpress MVP

 

For clinicians, the findings suggest that having accurate diagnostic information available during the patient encounter can improve treatment decisions and reduce uncertainty.

Clinicians Who Experienced Xpert Xpress MVP# Reported Positive Experiences with Point-of-Care Testing

The study evaluated provider perceptions before and after implementation of point-of-care nucleic acid amplification testing (NAAT).

By the end of the trial:

  • 98% of providers were satisfied with test turnaround time
  • 85% were satisfied with workflow
  • 87% said they would like to incorporate point-of-care molecular testing in the future

 

These findings are notable because diagnostic accuracy alone is not sufficient for adoption. New technologies must also fit into real-world clinical workflows.

Vaginitis remains one of the most common reasons women seek gynecologic care, yet diagnosis can be difficult when symptoms overlap across multiple conditions, or when no infection is present at all.

The study# provides evidence that in clinics serving women with vaginitis symptoms, Xpert Xpress MVP  can help clinicians:

  • Increase the likelihood of appropriate therapy
  • Reduce unnecessary antibiotic and antifungal use
  • Deliver timely, actionable results to patients

 

The authors conclude that “Women who seek care for symptoms deserve accurate diagnosis and appropriate treatment.” The point-of-care testing approach with Xpert Xpress MVP can significantly increase the number of women appropriately treated compared to usual care.
