In Krankenhäusern in ganz Europa stellt die Infektion mit Clostridioides difficile (CDI) nach wie vor eine hartnäckige und kostspielige Herausforderung dar. Trotz jahrzehntelanger klinischer Erfahrung und immer ausgefeilterer Diagnostik bleibt es für Ärzt/innen und mikrobiologische Labore gleichermaßen eine Herausforderung, genau zu bestimmen, wer tatsächlich an einer CDI leidet und wer nicht. Im Kern des Problems liegt eine grundlegende Tatsache: diagnosing CDI is not a single test decision, but a clinical process that demands nuance, context, and stewardship 1,2.
Recent discussions show that even well-intentioned diagnostic strategies can lead to both underdiagnosis and overdiagnosis, directly impacting patient outcomes, infection control, and antibiotic use1.
Wenn Sensitivität nur eine Seite der Medaille ist
Molekulare Tests, insbesondere NAATs, haben die CDI-Diagnostik verändert. NAATs offer greater sensitivity than toxin enzyme immunoassays or GDH screening, making them central to many European testing algorithms3. However, factors beyond sensitivity also require attention.
Sensitivität allein garantiert keine klinische Sicherheit. Ein positives molekulares Ergebnis bestätigt toxigene C. difficile, weist jedoch nicht immer auf eine aktive Infektion hin. Eine asymptomatische Besiedlung ist im Gesundheitswesen weit verbreitet. Without careful interpretation, sensitive tests may identify the organism even when it is not causing disease4,5.
Die verborgenen Fallstricke mehrstufiger Algorithmen
Um Genauigkeit und Praktikabilität in Einklang zu bringen, stützen sich viele Labore auf mehrstufige diagnostische Abläufe, die GDH, Toxintests und NAATs kombinieren. While these algorithms aim to improve predictive value, they introduce their own vulnerabilities 6,7.
Ein GDH-Screening kann beispielsweise sowohl falsch-negative als auch falsch-positive Ergebnisse liefern. Einige Clostridium-Arten und sogar nicht verwandte Organismen können zu Kreuzreaktionen führen, während bestimmte toxigene Stämme möglicherweise völlig übersehen werden. Toxin assays, meanwhile, are prone to variability and can generate misleading results when used in isolation6,8.
Bestimmte Stämme stellen traditionelle Annahmen infrage. Einige Isolate von C. difficile weisen keine Toxine A oder B auf, tragen aber Gene für binäre Toxine. Without accounting for these nuances, diagnostic tools may miss or delayed clinically significant cases 6,9,10.
Was uns die Erkenntnisse aus der Praxis zeigen
Neue klinische Daten verändern die Sichtweise der Labore auf Teststrategien für CDI. Studien, in denen Reverse-Two-Step-Algorithmen und eigenständige NAAT-Verfahren untersucht wurden, zeigen konkrete Vorteile: fewer hospital onset CDI cases, reduced antibiotic exposure, and more consistent infection control practices5,11.
Auf der Grundlage dieser Erkenntnisse ist es eine bemerkenswerte Erkenntnis, dass einige NAAT-positive Patient/innen beim GDH-Screening möglicherweise negativ getestet werden. Clinically, using GDH alone as a gatekeeper may result in missed diagnoses, allowing unidentified cases to contribute to hospital transmission5.
Umgekehrt birgt die Behandlung jedes molekular positiv getesteten Patienten ohne Berücksichtigung von Symptomen oder Risikofaktoren die Gefahr eines unnötigen Antibiotikaeinsatzes. Dies unterstreicht eine zentrale Botschaft: laboratory results must be interpreted through a clinical lens, not in isolation4,12.
Verantwortungsvoller Umgang ist ein Muss
Mit leistungsfähigeren Diagnostikverfahren wächst auch die Verantwortung, sie gewissenhaft einzusetzen. Diagnostic stewardship, the principle of ordering the right test for the right patient at the right time, is now recognized as an essential partner to antimicrobial stewardship1,2.
Im Rahmen von CDI bedeutet verantwortungsvolles Handeln, dass man in Situationen mit geringer Wahrscheinlichkeit auf vorschnelle Tests verzichtet, wiederholte Untersuchungen ohne klinische Indikation vermeidet und Entscheidungshilfen in die Workflows bei der Testanforderung integriert. Evidence from hospitals implementing stewardship have been associated with reductions in unnecessary testing and false‑positive results without clear evidence of increased missed diagnoses.13,14.
Dies ist insbesondere angesichts der zunehmenden Verwendung von gastrointestinalen Multiplex-Panels von Bedeutung. Diese Verfahren können zwar eine Vielzahl von Krankheitserregern schnell identifizieren, erhöhen jedoch auch die Wahrscheinlichkeit, dass eher eine Besiedlung mit C. difficile als eine Infektion festgestellt wird. Studies indicate that only a minority of panel positive results ultimately represent true CDI, yet many patients still receive treatment5,15.
Für die Zukunft: Integration statt Isolation
Die Zukunft der CDI-Diagnostik liegt nicht in einem einzelnen „perfekten“ Test. Instead, it depends on integrating advanced molecular diagnostics with clear institutional policies, clinical judgment, and stewardship frameworks 1,16.
Labore und Ärzt/innen müssen gemeinsam bestimmen, wann eine Untersuchung angebracht ist, wie die Ergebnisse zu interpretieren sind und wie die diagnostischen Abläufe mit den Zielen der Patientenversorgung im Einklang stehen. This integrated approach helps avoid harm, reduces unnecessary antibiotic exposure, and supports better infection control decisions2.
Letztlich geht es bei der Verbesserung der CDI-Diagnostik nicht nur um den Nachweis von Bakterien. Es geht darum, die Krankheit zu verstehen. And in an era of powerful diagnostics, that understanding has never mattered more1.
